Target Audience

This course is for staff with responsibilities for assessing the needs of a customer and planning their support.

 

Aims

This course aims to provide you with the knowledge, tools and techniques to implement good practice in assessment, support planning and review.

 

Learning Outcomes

At the end of the course you will know the principles and theory relating to good practice in assessment, support planning and review.  You will understand the importance of promoting a person centred approach and will have the opportuniy to put your knowledge into practice using case studies and real life examples.

 

Course Contents

  • The importance of a holistic approach to assessment and support planning
  • Legislation, organisational requirements, codes of practice, standards and guidance relevant to developing, implementing and reviewing care plans
  • Person centred support planning including how to support the individual to lead the process
  • Theories relevant to support planning including the support planning cycle and SMART goals
  • The importance of considering a customer's strengths and aspirations as well as needs
  • Recording the plan in a suitable format
  • Monitoring, evaluating and reviewing the support plan

This course links to QCF Unit HSC 3020; Facilitate person centred assesssment, planning, implementation and review.  You may use your learning from attending this course torwards the Level 2 and 3 Health and Social Care
